Request for Services
The Request for Services form allows the Conference Services staff to initiate services. This completed form, submitted to the Conference Services office is a request for services. It does not guarantee availability nor assure reservations as requested but allows the Conference Services staff to initiate services. The completed Request for Services form will assist in generating the Conference Services Agreement, when executed, will confirm and guarantee reservations.
